I have my yarn on consignment at my LYS, and we just did a verbal agreement. But since I'm up there a few times a month, I'm comfortable with that. If I were to put items on consignment elsewhere, I'd want a more formal agreement. (How formal is probably up to what you're most comfortable with.)
