Personally, I don't gather any information from the people using my site. But I'm not sure whether my provider does. The guy on the support line said something about Web Analytics but that's seems to be a generic term, not a specific company…

As far as I am aware, there is no specific wording that needs to be included. The privacy policy and terms & conditions do need to be there, but how those policies are worded is up to you.
